Healthy Families is a nationally accredited, statewide program that works to prevent child abuse and neglect before it begins. Established in Florida Statute 409.153, Healthy Families plays a critical role in identifying, funding, and supporting initiatives that improve the development and life outcomes of children while preserving and strengthening families, with a primary focus on prevention. The program provides community-based home visitation services, promoting child health and development as well as positive parent-child interactions. In addition to Parent Education & Support, Healthy Families offers Mental Health Counseling to participants experiencing Postpartum Depression or other mental health challenges. Families join the program either prenatally or shortly after birth and can remain in the program for three to five years, depending on their needs.

The Family Support Worker (FSW) supports families residing in Alachua, Columbia, Union, and Bradford counties by helping them improve family functioning and raise healthy children. This role involves regular in-home visits to provide advocacy and support for personal and parenting needs. Initially, visits occur weekly and taper off to every two to four weeks as families make progress toward their goals. The Family Support Worker is responsible for managing a caseload of 15-25 families, ensuring ongoing programmatic compliance with both University of Florida and Healthy Families of Florida standards. About This Role
 
Establishes a trusting relationship with the client and family by maintaining regular home visit as required per client level.  Maintains dependable, punctual work schedules with a minimum of 80% completed home visits per level requirement.  Assists strengthening parent/child interaction by providing in home education, utilizing established program curriculum and by modeling appropriate behavior during each home visit.
 
Travels to participant’s homes for regular home visits. Also travels to creative outreach meetings and various agencies for collaboration. Travels to the Healthy Family office for supervision, staff meetings and training.
 
Establishes a family support plan with goals, objectives, and activities with supervisor and client within 90 days of participant agreement. This plan directs every home visit and is reviewed and updated at least every 180 days. Administers Healthy Families America (HFA) tools at the required intervals for each family.
 
Administers Home Safety Checklists with all families, as per schedule, and provides associated teachings as required to include (but not limited to):  Shaken Baby, co-sleeping, water hazards, emergency preparedness, and poison prevention.  Prepare documentation of results for supervisor’s review, and files in client’s HFA chart. Demonstrates excellent knowledge of community resources, make referrals, and provides follow-up with families to insure needs are met. Meets with supervisors weekly (1.5 hours minimum) for case management of all families on caseload.  Documentation of observations, interactions, and teaching completed at each home visit must be completed and submitted to supervisors within 72 hours of making home visit.  All other forms must be completed on time per written protocol.
 
Attends regular staff meeting and case conferences as required.  Participates in continuing education training and shares knowledge with other team members.  Assists and support ongoing program growth and development. Performs QA audits for families as directed by supervisor to update family data. Performs other duties as required to meet program objectives.

Regular Work Schedule

  • Daily, Monday through Friday, 08:00 AM to 04:30 PM
  • Total hours per week: 40
  • Some flexibility may be required to accommodate home visits (after 05:00 PM and/or on weekends)
 
Expected Salary:

$16.00 hourly

Minimum Requirements:

High school diploma or equivalent and one year of experience working with children or families.

Valid driver license and automobile insurance.

  • Ability to establish trusting relationships
  • Experience in working with or providing services to children and families
  • Acceptance of individual differences 
  • Experience and willingness to work with the culturally diverse populations which are present among the  program’s target population
  • Knowledge of infant and child development
  • Open to reflective practice (e.g., has capacity for introspection, communicates awareness of self in relation to others, recognizes value of supervision, etc.)
