The Ambulatory Care Administrator III provides clinical care to patients at Archer Family Health Care and serves as the lead for the newly developed Chronic Care Management (CCM) program. This position is responsible for providing strategic direction, clinical oversight, and operational leadership to ensure the delivery of high‑quality, patient‑centered care for individuals with chronic conditions.
Key responsibilities include evaluating and enhancing care delivery processes, optimizing resource utilization, mentoring and educating staff, and establishing care standards tailored to chronic disease populations. The role will collaborate closely with clinic staff and providers to collect and analyze patient data, develop individualized care plans, and support improved health outcomes. The position will also monitor quality improvement measures, ensure accurate documentation, and maintain compliance with all regulatory requirements related to CCM. Additionally, the Ambulatory Care Administrator III will contribute to system‑wide initiatives aimed at strengthening chronic care delivery.
Leadership, Program Oversight, and Clinical Management
Provide day to day leadership and supervision for CCM services, including delegation of responsibilities, coordination of care activities, and oversight of team performance.
Lead the planning, development, implementation, and evaluation of the CCM program, ensuring alignment with organizational goals and evidence based best practices.
Promote shared goals and a team based approach to care delivery to support high quality clinical outcomes for complex chronic disease populations.
Conduct comprehensive clinical assessments, collect and analyze patient data, identify care gaps, and make evidence based recommendations for individualized patient care plans.
Evaluate the effectiveness of nursing care, monitor patient progress, and document all clinical interventions and patient responses in accordance with professional standards.
Demonstrate a strong commitment to patient safety through early identification of safety risks, reporting of adverse events, and active participation in creating safer clinical processes.
Identify population specific needs within ambulatory care settings and lead the development of standards of care, patient/family education materials, and staff training resources tailored to chronic disease management.
Serve as a clinical expert and resource for staff regarding chronic care standards, CCM workflows, regulatory requirements, and evidence based practice.
Quality Improvement, Operations, and Resource Management
Lead or participate in quality improvement initiatives, operational assessments, and organizational projects related to chronic disease management, care coordination, or ambulatory operations.
Contribute to the evaluation and enhancement of care delivery models through analysis of performance metrics, outcome data, and feedback from staff and patients.
Ensure proper utilization, care, and stewardship of clinical supplies, equipment, and material resources needed to support CCM and ambulatory operations.
Develop process improvements that streamline workflows, improve efficiency, and support compliance with regulatory and payer requirements for CCM services.
Education, Training, and Professional Development
Design, implement, and maintain orientation programs for new clinical staff involved in CCM and ambulatory care.
Precept students in the health professions, providing structured learning experiences, mentorship, and performance evaluations.
Provide ongoing training, coaching, and competency development for team members involved in chronic care management and ambulatory services.
Support professional growth by facilitating educational sessions, encouraging participation in continuing education, and fostering an environment of lifelong learning.
Clinic Support, Patient Check-In, and Nursing Coverage
Assist LPNs with the patient check in process, including rooming patients, obtaining vital signs, performing preliminary assessments, and preparing patients for provider evaluation when necessary.
Provide nursing coverage on an as needed basis to support clinic operations, maintain continuity of care, and ensure smooth patient flow during staffing shortages or high volume periods.
Collaborate with clinical support staff to ensure efficient coordination of patient care, timely documentation, and adherence to clinic protocols
